No matter what cane or crutch is chosen it is important to get the right one that meets the needs of the patient. If you are the patient, you may already know what you need or what you would like better. If you are taking care of the patient, listening to that person and letting them tell you what they need is a good first step. They may have to try out some options until they find what works best for them, but they should be active participants in their care for as long as they are able. By keeping them active in the decision making process they feel more useful and the transition from their previous abilities to the need for a crutch or cane can be made in a less painful way. Depression can be a real problem for the elderly or for people who are disabled and allowing them to have as much control over their lives as possible works to lessen that.

Reasons why your wound or bed sore isn't healing

Most skin wounds heal fairly quickly…in a matter of days or sometimes weeks. The body usually takes care of all the problems that can cause a wound to not heal.

But why is your wound not healing now?

There are a number of reasons for this, such as;
. Medical problems (like diabetes)
. Medication which can delay healing
. Inadequate nutrition and hydration
. Inadequate blood supply
. Bacteria
. Pressure on the area
. The changing nature of the skin as we age

But even with any or all of these issues, treatments today can overcome many of these problems, leading to a closed wound.

A bit more about wounds…

There are 3 categories of wounds that you might want to understand. The approach and treatment to each of these 3 categories can be greatly different.

1) Acute wounds
2) Chronic wounds
3) Other wounds, like skin cancers
. We are not covering burns in this booklet

Acute Wounds
An acute wound is generally thought of as a ‘new’ wound, usually from a trauma or even a surgical incision. A trauma can be something like a cut or scrape.

As you might realize, an acute wound starts from the outside of the skin and goes into deeper tissues. This is an important concept because a ‘chronic wound’ can be the reverse, starting inside the body and ultimately breaking down the skin from the inside.

Acute wounds usually heal quickly, but can turn into non-healing wounds for a variety of reasons. If your acute wound hasn’t healed or showed the usual improvement within a few weeks, it would be wise to consider it a chronic or problematic wound. You would probably benefit from expert help. Obviously, if you are seeing any signs of infection, you need immediate medical treatment.

Chronic wounds
Chronic wounds can be thought of in these 2 ways:
a) An acute wound that hasn’t healed after a certain amount of time OR
b) A wound associated with certain problems or conditions, like a pressure ulcer,
diabetic ulcer, venous stasis ulcer or arterial ulcer.
Dealing with these wounds involves a proper workup and diagnosis, followed by a flexible treatment plan which can change as the wound starts improving. The good news is that many of these wounds can be healed, no matter how tough. Your wound
clinician should have an expectation of progress and be able to spot if the wound is healing properly or not.

Other wounds
This is the category where things get tricky. Sometimes, you have a break in your skin and you don’t really know how it happened. Sometimes, you have a wound that you think is a chronic wound, but it isn’t responding to treatment. These wounds might not be the ‘typical’ wounds that we spoke about in the sections above. Maybe
a biopsy needs to be taken to make sure it isn’t a different type of wound, like a skin cancer.

What has changed in wound care recently?
What used to be the standard-of-care, is no longer the standard-of-care when it comes to wounds. New understanding of the way a wound heals has led to breakthroughs in new product development, along with an understanding of products which should no longer be used on skin wounds.

For example, keep it open to air has been replaced by a concept called moist wound healing. While topical ointments are still used, advances have led to new topical products which are superior in many ways to the older products.

The common ‘saline or Betadine wet-to-dry’ treatment is now understood to not promote healing and is reserved for specific clinical situations. Surgical procedures can now be performed to
either close the wound immediately or correct issues in the body, such as poor blood flow, that contribute to the non-healing nature of the wound.

In addition, a variety of new technologies are now on the market specific to wound care, including things like: nanocrystalline silver, growth factors, tissue engineered products, MMP inhibitors, enzymatic debriders, tissue scaffolds, topical negative pressure and collagen based products.

Is every doctor or nurse aware of these advances in wound care?

Absolutely not. As you would expect, clinicians who don’t focus on wounds might not be aware of the latest products and protocols, or how best to use them. While they are well meaning and they base their decisions on past experience, your situation might be one best
served by a wound specialist. If you or someone you know has a non-healing wound,

you most likely need to be seen by a wound specialist to stand your best chance of getting better, faster.

Wound centers typically have three things that should be important to you. Without these three things, wound closure will be difficult.

1) Up to date knowledge on wound issues
2) Easy access to the right testing equipment
3) Easy access to the right products

What can I do myself, as far as treatment?

Here’s the bad news. There is no magical cure or single product for chronic wounds.

Whatever you read on the Internet, there isn’t one simple remedy for all chronic wounds. Often, wound specialists use a certain product for a limited amount of time, switching to a completely different product as the wound starts to improve and has different requirements.

Specialists should also be looking at your blood flow, your nutrition and other factors which play a role in wound healing. If you have a non-healing wound, a chronic wound or a wound that could be put into the ‘other’ category’, you really need to see a wound specialist for an appropriate diagnosis and treatment plan.

Anything else you do will most likely be guesswork on your part, resulting in extra expense on potentially ineffective products, losing time in the process and maybe allowing the wound to get worse.

If financial issues are causing you to self-treat, it would be a good idea to speak with your local wound specialist about your financial hardship to see what can be done.

But when it comes to wound care, there are many people who call themselves ‘wound experts’. Unfortunately, it’s common to see clinicians prescribe a different treatment protocol for the same wound an issue that has been discussed extensively within
the wound care community.

When it comes to wound care, you’ll find that there are legitimate wound specialists in each of the following medical fields:
. Physicians
. Surgeons
. Nurse Practitioners
. Physician Assistants
. Nurses
. Physical Therapists

As the patient, how are you to know which one to see, which one is going to give you your best chance at healing? It’s confusing to say the least.

Heat and cold therapy are ancient forms of treatment that still hold value today. The premise behind both heat therapy and cold therapy is that of a very basic, natural treatment that is effective on many problems. The age-old debate, of course, is whether to use heat or cold on an injury to promote healing. Before deciding on an appropriate therapy, it is helpful to understand the underlying differences between each type of therapy.

The heat therapy solutions that most of us are familiar fall into the category of superficial heat therapy. Superficial heat therapy works by warming the skin and the underlying tissue directly, making it an ideal treatment for sore muscles and back pain. As the skin and tissue warm, the blood vessels dilate, increasing blood flow, decreasing pain and delivering additional nutrients to the injured area. This in turn promotes the healing process. In addition, the heat increases the flexibility of the tissue, leading to a decrease in stiffness and soreness associated with injuries.

Heat therapy is most often used to relieve chronic injuries or newer injuries that are not red or swollen. Heat therapy can also be an extremely effective treatment for chronic pain when applied to sore muscles and ligaments prior to exercise. While heat is soothing on arthritis, it can sometimes cause additional swelling or pain. Lastly, heat therapy is a very effective treatment for muscle spasms or tightness, as the heat relaxes muscles and encourages greater muscle elasticity. Because heat dilates the blood vessels, heat therapy should never be used if the injury is inflamed, as the heat will exacerbate any swelling. Heat therapy is discouraged if an individual has diabetes, dermatitis, peripheral vascular disease or an open wound.

Forms of superficial heat therapy run the full spectrum, from very simple to more complex. On the simpler side, heat therapy products include items that everyone has on hand. Incredibly easy to use and very inexpensive, these treatments include hot water bottles, warm baths and heating pads. In addition, there are many simple solutions available that radiate heat longer and more uniformly. Heat packs range from a simple compress, such as the SensaFlex Hot and Cold Compress, to more comprehensive treatments that wrap around the injured area, such as the Therma-Wrap Hot and Cold Compresses. Paraffin baths can also be used to surround a sore or strained body part, such as a hand, foot or elbow. Similarly, cold therapy has many of the same benefits as heat therapy. Cold therapy, also known as cryotherapy, acts as a better numbing agent than heat, anesthetizing the sore tissue and decreasing pain. The cold relieves swelling by constricting the blood vessels and reducing circulation, which in turn also reduces internal or external bleeding.

Cryotherapy is most effective when used directly after an injury. A sharp sprain or typical sports injury is generally best treated with cold therapy for 10-15 minutes at a time. Cold therapy can also be a useful treatment for a chronic injury, when the pain is recurring after a certain activity or event, such as a tender ankle after running. If there is any swelling or redness present, cold therapy is always the preferred treatment. There are, however, some situations in which cold therapy should never be used: if you suffer from rheumatoid arthritis, Raynaud’s syndrome, have a cold allergy, impaired sensation or any paralysis.

Cold therapy techniques are generally simple, straightforward and inexpensive. Homemade cold treatments can include ice bags, ice massages or a frozen bag of peas. Slightly more thorough and portable treatments include cold packs and ice packs in a variety of sizes and shapes to conform to your body, from the child-size Boo Boo Pac to adult-size cold compresses (ColPac Vinyl Therapy Pack and Therma Wrap Hot and Cold Therapy).

Both treatments have a few commonalities that make them appealing. The cost and convenience, added to the fact that these are completely natural treatments, add to the appeal. All that is left to do is decide on your chosen therapy.

I'm here today to talk about what is but should not be a touchy subject.

Most of us were taught by our parents, and so we taught our children, that wetting the bed is something to be ashamed of. I'm not here to debate how children should be raised, but rather to raise conciousness regarding the medical problem of incontinence in adults.

When you were a toddler your parents taught you to control your bladder so you could function in society, and they were taught by their parents that they should be ashamed of themselves for peeing in bed.

Because this has been so ingrained in our minds, our parents most likely believe that they should be ashamed of themselves for their recent inability to control their own bladder function.

Ridiculous isn't it? When you think about it.

But true, and with potentially deadly consecquences.

Incontinence is the loss of control over the urination process.

Urine is acidic, it breaks down the skin.

In the elderly one of the most life threatening things that can happen is the opening of sores or wounds also called ulcers. An open wound is susceptible to infection and this danger is multiplied in a hospital or nursing home setting where there are many sick people with infections,like staff, floating around. Viral infections like staff kill people every day.

Lying in pee soaked bedding or sleeping in pools of urine on a waterproof mattress is going to quickly open a sore on the body of an elderly person who already has a weak immune system.

You must be prepared to act quickly when incontinence occurs.

This requires you and your parent to be mentally prepared to broach the subject immediately when loss of bladder control first happens.

That safety feature is important to a lot of caregivers but it’s not something that can be found on every APM available on the market today. Another great feature of this brand of mattress is the anti-microbial cover. It is non-shear and fully encloses the entire mattress and the three-inch foam base.

The Masonair brand of alternating pressure mattress is dedicated to both comfort and quality. Physical therapy, however, does not have to have such a stigma about it. It is a necessary part of life for a lot of people and those people should not be ashamed of working hard to return to the lives that they led before.

For an elderly person physical therapy can be especially difficult, and the same is true for people who are handicapped. These groups may never have – or return to, in the case of an elderly individual – the same mobility level that is seen in most of the population. Even though this is a reality that a lot of people face, that does not mean that they have to waste away and not enjoy everything that they are able to do. There are a lot of different kinds of mattress overlays and it depends on exactly what you’re in need of as to which one would be best for your situation. The most common of the overlays are filled with water, air, or gel. All three have their benefits for the bed-bound patient or for a person who is mobile but who does spend the majority of his or her time in bed. If you are the caregiver for a person in this condition – whether that person is a relative or someone you are being paid to care for – you want that person to be comfortable and well-cared-for. There are a lot of ways to do this, and one of the best is through the use of a mattress overlay. But which one should you buy – water, air, or gel?

A popular water overlay is the Hydr-O-Ease Flotation Mattress Overlay. These can be purchased in both gel and water varieties for about $76 or $62 respectively. This particular overlay works well for home care, but it can also be used in nursing homes and in hospitals, both for therapeutic treatment and for pressure relief. The sleeping surface is comfortable and there is one big benefit with this overlay – it doesn’t have that wave effect and rocking problem that is usually found in most water mattresses. It also won’t easily leak because the seams are heat sealed and then electronically tested. The overlay is durable, too, being composed of high-grade vinyl that has been treated with an anti-microbial agent. Clean-up and sanitation is a snap. The pad measures 32 inches by 72 inches and the instructions for filling it and taking care of it are printed on it, to avoid losing an instruction sheet. There are fill lines on the water flotation overlay and pre-measured packets for the gel overlay to make sure that it is filled to the proper depth. Another good feature of the Hydr-O-Ease is the temperature gauge, which is easy to read and can help avoid problems like hypothermia that can come about from allowing a patient to lie on a mattress full of cold water or gel.

Another popular choice is the Air-O-Ease Static Air Mattress Overlay. This overlay avoids the chance of leakage that can come from a water or gel mattress. It has the same heat sealed seams and anti-microbial surface as the Hydr-O-Ease and costs a bit less, at around $52 making it a more affordable option for patients who are on a very fixed income. The air circulation that comes through the unique construction of this Mason Medical air mattress helps to keep the buildup of heat down and also lower the interface pressures that patients have when they spend a long time lying in their bed. It also helps to remove moisture to keep a patient dry. Placement over the mattress is secured with retraining straps that can be adjusted to fit all types and sizes of mattresses, preventing the overlay from sliding around. The Air-O-Ease is the same size as the Hydr-O-Ease, measuring 32 inches by 72 inches, and features two adaptors for filling the mattress and removing the air when the mattress is no longer needed. One of the best features of this overlay is that it conforms to California Technical Bulletin #117, which means that it has a high degree of fire retardancy. It ships free to anywhere in the continental United States.

If a gel overlay would be preferred, the Sierra Gel Mattress Overlay might be the right choice. At just under $223 it is more expensive than the other options but some people find it to be superior to the other options available for mattress overlays. It is slightly bigger than its competitors, measuring in at 34X78X3, and it not only meets the same fire retardant specifications of the Air-O-Ease but also meets all standards for infection control. It offers a support base and a shell made of a high-density polyurethane foam. Inside the overlay are a series of gel bladders. The surface cleans easily and there is no assembly or inflation necessary. The gel is non-toxic should it ever leak due to a puncture or other problem, and the anti-microbial and anti-bacterial border material and bottom panel help to keep the patient both comfortable and healthy.

When purchasing a pad or overlay for a mattress there are a lot of different considerations and it is easy to see here that there are also a lot of different options. Most of these depend on issues like cost and features, but personal preference also plays a strong role. Patients who are able should certainly have a say in what type of overlay is purchased. For those who do not have that ability, taking a good look at features and benefits can go a long way toward making the right overlay choice.

In addition to the five-year compressor warranty that the 5-Liter model has, the 10-Liter model also offers a warranty of three years on all of the other components of the machine.

The sound level of the 10-Liter model is just 8 dB louder and the flow rate is the same, at 87% to 95%. Both operate in the same way, need the same kind of maintenance, and have the same basic dimensions. The difference between the two machines and which one a person would want to buy would come down to how much oxygen that person needed on a minute by minute basis. Needing a flow rate of 10 liters per minute would require the larger and more expensive model, as the smaller one wouldn’t be able to produce that amount. After all, although I have been through my share of hard times, I have always prided myself on being independent. I had always thought that drug and alcohol rehab was for wimps.
